Background Although the importance of clinical ethics in contemporary clinical environments is established, development of formal clinical ethics services in the Australia health system has, to date, been ad hoc. This study was designed to systematically follow and reflect upon the first 18 months of activity by a newly established service, to examine key barriers and facilitators to establishing a new service in an Australian hospital setting. Methods: how the study was performed and statistical tests used A qualitative case study (...) approach was utilised. The study gathered and analysed data using observations of service committee meetings, document analysis of agendas and minutes, and semi-structured interviews with committee members to generate semantic themes. By interpreting the thematic findings in reference to national capacity building resources, this study also aimed to provide practice-based reflections for other health agencies. Results: the main findings An overarching theme identified in the data was a strong commitment to supporting clinicians facing difficult patient care decisions and navigating difficult discussions with patients and families. Another key theme was the role of the new clinical ethics support service in providing clinicians with a pathway to raise system-wide issues with the organisation Executive. While there was strong clinical engagement, consumer and community participation remained a challenge, as did unresolved governance issues and a need for clearer policy relationship between the service and the organisation. Conclusions: brief summary and potential implications Considering these themes in relation to the national capacity building resources, the study identifies three areas likely to require ongoing development and negotiation. These are: the role of the clinical ethics support service as a link between the workforce and the Executive; the incorporation of consumers and patients; and ethical reasoning. To improve the effectiveness of the service, it is necessary to increase clarity on the service’s role at the governance and policy level, as well as develop strategies for engaging consumers, patients and families. Finally, the capacity of the service to reflect on complex cases may be enhanced through explicit discussions of various different ethical frameworks and ways of deliberating. (shrink)

Nesse sentido, (...) ao trabalhar com a imbricação entre a materialidade verbal e não verbal, busco lançar olhar sobre duas obras do artista zimbabueano Dan Halter, considerando, desse modo, os efeitos de sentido que deslizam metafórica e metonimicamente para outros sentidos do corpo que se desdobram em diferentes imagens do sujeito, tendo em vista os processos de estruturação do conflito e da tensividade do/no social a partir dos espaços ocupados por estes corpos e atravessados que são pelo simbólico, pela ideologia e pela história. To this end, we propose a theoretical-analytical dialogue between the dialogical perspective developed by Russian Mikhail Bakhtin and the historical-materialism perspective, based on the theoretical assumptions of the French Discourse Analysis, in order to analyze the object of this study in its relation with body, memory, and discourse. In this sense, by working with the imbrication between verbal and non-verbal materiality, we intend to focus on two specific works by Zimbabwean artist Dan Halter, considering the meaning effects that slide metaphorically and metonymically to other senses of the body that unfold in different images of the subject, in view of the processes that structure the conflict and tensivity of the social from the spaces occupied by these bodies and crossed by the symbolic, by ideology, and history. This article seeks through Ejagham women’s experience in the ritual dances of Ngbokondem and Moninkim to engage the notion of patriarchal control of African women’s sexuality in ‘female genital mutilation’ discourses as postulated by second-wave feminist theorists such as Daly, Koedt, Hosken and so on. A firmly based patriarchy threatens culture, sexuality and identity; the article shows how women use varied coping mechanisms, including aid schemes, sexual insurgency and even breaking of bodies to define their place and identity in a (...) patriarchal society. However, some Ejagham women’s practices are themselves laced with patriarchal violence where women postulate as main participants. The article coins the term ‘triple patriarchy’ to capture this phenomenon. It portrays how Ejagham women, who are interrogating domination, also act in ways that reinforce patriarchy. Using womanist theological lens as the hermeneutic framework, the article concludes that women initiation practices are the foundations through which women grasp the meanings of Christian initiation rituals such as baptism, eucharist, marriage and so on. Hence, they should not be destroyed but fine-tuned; for there is more about the women’s practices that need to be engaged than labelled. (shrink)

Can I?This 1966 fragment issues directly from, and is a preternatural reminder of a passionate problematic—questions of gender, in relation to an engagement with poetry, poetry figured as a set of psychosocial and intellectual institutions imbuing a practice of writing in lines. This fragment knew and represented issues, points of political and cultural contradiction about the construction of authorship and the poetic career before their maker could see them as such. The fragment was a cognitive act, not just in autobiography. It functioned as a probe of a situation then almost invisible and always underacknowledged; to have understood it then, one would have needed the cultural apparatus precipitated (though not elaborated) by a social movement—second wave feminism, something that had not yet occurred.Despite mirroring each other in a dramatic hinged appositive, with Lady as hinge (“I, Lady, you”), the female “I” is separated by the very concept “Lady” from her addressee, one traditional addressee of poems, the female “you.” “Lady” is both hinge and barrier; she belongs to “my true love”—a charming and smug ballad turn, filled with idealizations of class and sexuality. Yet the “I” wants to hold this “you” figure to herself, actualizing in female-to-female attraction the long half-life of a Petrarchan structure of feeling in order to create female authorship. Now that gesture could be interpreted as desire (in a [End Page 31] lesbian, matrifocal, or preoedipal plot), or as self-aggrandizing forms of egoistic desire. Is the desired embrace regressive? immature? narcissistic? self-serving? greedy? masturbatory? (all these having been thought of as bad attitudes with a bad press—one might now wonder). Fueled by an ambisexual or polysexual suggestiveness, the poem makes a wobbly declaration of vocation in its desire to embrace singular, hieratic woman herself, yourself, myself. Yet as pronouns multiply, a serious problem is accosted. “But you are myself. Can I?”The fragment stops, startled, where the cultural problem began. Authorship was compromised and almost aborted; in any event, these issues, which seemed to me, in my (then commonplace) ignorance of female cultural production, to have no analogues, pointed me toward total blockage. The “can I” is not simply a personal questioning of my own powers, but concerns general contingencies of gender and female place in culture, a then invisible social horizon. Am “I” forbidden to poetry by one—but one key—law of poetry—the cult of the idealized female? Should “Lady” be ditched totally? or examined? both? and how to poke at a Petrarchanism that seems utterly belated, never uniformly hegemonic, and not, after all, the whole content of poetry? (As I bear down upon it, it recedes, is evasive.) Yet any number of writers do take “Lady” for granted as one of the institutional assumptions of poetry. In this book, Tim O'Keefe reconstructs the theory of freedom of the ancient Greek philosopher Epicurus (341-271/0 BCE). Epicurus' theory has attracted much interest, but our attempts to understand it have been hampered by reading it anachronistically as the discovery of the modern problem of free will and determinism. O'Keefe argues that the sort of freedom which Epicurus wanted to preserve is significantly different from the 'free will' which philosophers debate today, and that in its emphasis on rational action it (...) has much closer affinities with Aristotle's thought than with current preoccupations. Calvin Blanchard (1808–1868) was a prolific, albeit repetitive, author, publisher, and printer who was identified by L. L. and Jessie Bernard as an early American sociologist who helped introduce Americans to Auguste Comte. The Bernards also said that Blanchard was mentally ill.1 The Bernards’ discussion of Blanchard is the only recognition of Blanchard that predates the reprinting of one of his novels, The Art of Real Pleasure (1864), in Arthur O. Lewis’s 1971 American Utopian Literature set of forty-two volumes. (...) In this paper, I provide an easy road to nominalism which does not rely on a Field-type nominalization strategy for mathematics. According to this proposal, applications of mathematics to science, and alleged mathematical explanations of physical phenomena, only emerge when suitable physical interpretations of the mathematical formalism are advanced. And since these interpretations are rarely distinguished from the mathematical formalism, the impression arises that mathematical explanations derive from the mathematical formalism alone. I correct this misimpression by pointing out, in the (...) cases recently discussed by Mark Colyvan, exactly where the interpretations of the formalism were invoked and the function they played in the resulting explanations. A viable form of easy - road nominalism, which is also sensitive to scientific practice, then arises. (shrink)

Obesity is the focus of multiple lines of inquiry that have -- together and separately -- produced many deep insights into the physiology of weight gain and maintenance. We examine three such streams of research and show how they are oriented to obesity intervention through multilevel integrated approaches. The first research programme is concerned with the genetics and biochemistry of fat production, and it links metabolism, physiology, endocrinology and neurochemistry. The second account of obesity is developmental and draws together epigenetic (...) and environmental explanations that can be embedded in an evolutionary framework. The third line of research focuses on the role of gut microbes in the production of obesity, and how microbial activities interact with host genetics, development and metabolism. These interwoven explanatory strategies are driven by an orientation to intervention, both for experimental and therapeutic outcomes. We connect the integrative and intervention-oriented aspects of obesity research through a discussion of translation, broadening the concept to capture the dynamic, iterative processes of scientific practice and therapy development. This system-oriented analysis of obesity research expands the philosophical scrutiny of contemporary developments in the biosciences and biomedicine, and has the potential to enrich philosophy of science and medicine. In this book Anthony O’Hear examines the reasons that are given for religious faith. His approach is firmly within the classical tradition of natural theology, but an underlying theme is the differences between the personal Creator of the Bible or the Koran and a God conceived of as the indeterminate ground of everything determinate. Drawing on several religious traditions and on the resources of contemporary philosophy, specific chapters analyse the nature of religious faith and of religious experience. They examine connections (...) between religion and morality, and religion and human knowledge – the cosmological, teleological and ontological arguments, process thought, and the problem that evil presents for religion. The final chapter returns to the inherently dogmatic nature of religious faith and concludes that rational people should look beyond religion for the fulfilment of their spiritual needs. (shrink)

Combating homophobia, racism, sexism, and other forms of discrimination and violence in our society requires more than just focusing on the overt acts of prejudiced and abusive individuals. The very intelligibility of such acts, in fact, depends upon a background of shared beliefs, attitudes, and behaviors that together form the context of social practices in which these acts come to have the meaning they do. This book, inspired by Wittgenstein as well as feminist and critical race theory, shines a critical (...) light on this background in order to show that we all share more responsibility for the persistence of oppressive social practices than we commonly suppose—or than traditional moral theories that connect responsibility just with the actions, rights, and liberties of individuals would lead us to believe. First sketching a nonessentialist view of rationality, and emphasizing the role of power relations, Peg O’Connor then examines in subsequent chapters the relationship between a variety of "foreground" actions and "background" practices: burnings of African American churches, hate speech, child sexual abuse, coming out as a gay or lesbian teenager, and racial integration of public and private spaces. These examples serve to illuminate when our "language games" reinforce oppression and when they allow possibilities for resistance. Attending to the background, O’Connor argues, can give us insight into ways of transforming the nature and meaning of foreground actions. (shrink)

Many modern conservatives and feminists trace the roots of their ideologies, respectively, to Edmund Burke and Mary Wollstonecraft, and a proper understanding of these two thinkers is therefore important as a framework for political debates today. According to Daniel O’Neill, Burke is misconstrued if viewed as mainly providing a warning about the dangers of attempting to turn utopian visions into political reality, while Wollstonecraft is far more than just a proponent of extending the public sphere rights of man to include (...) women. Rather, at the heart of their differences lies a dispute over democracy as a force tending toward savagery or toward civilization. Their debate over the meaning of the French Revolution is the place where these differences are elucidated, but the real key to understanding what this debate is about is its relation to the intellectual tradition of the Scottish Enlightenment, whose language of politics provided the discursive framework within and against which Burke and Wollstonecraft developed their own unique ideas about what was involved in the civilizing process. _The purpose_ of this study is the reflection on ways of philosophical legitimation for the "Self-made-man" idea in the worldview of the Renaissance and Reformation. _Theoretical basis._ Historical, comparative, and hermeneutic methods became the basis for this. The study is based on the works of Nicholas of Cusa, G. Pico della Mirandola, N. Machiavelli, M. Montaigne, E. Roterodamus, M. Luther, J. Calvin together with modern researchers of this period. _Originality._ The analysis allows us to come to the conclusion that casts (...) doubt on the still widespread belief according to which the emergence of the "Self-made-man" idea is localized by the process of forming the American cultural code and the ideological heritage of Benjamin Franklin. It is highlighted that the formation of this idea is the result of a long process that originated in the ancient world and gains a special impetus in the Renaissance and Reformation. Precisely in the cultural context of the latter, the ancient intention to recognize the individual’s right to self-determination and self-government, which in the depths of Christian theology acquires only a potentially universal character, becomes not only acceptable but also, in the context of Protestantism worldview, the only admissible, in fact, individual’s obligatory life guidelines. _Conclusions._ Humanistic and reformation thoughts quite naturally led to further ideological legitimation of the person’s idea of who is creating oneself. This legitimation was during the complex interaction of numerous factors of culture in the Late Middle Ages, as well as ideas and intentions inherited from Antiquity. Key among them was the gradual formation of a new social order, in essence, indifferent to paternalistic rudiments, together with the ethics of Protestantism corresponding to it. The latter does not only legalize but, de facto, sacralizes the individual’s reorientation from hopes for the synergy of God’s grace and own free will in personal salvation, toward the self-reliance and personal efforts, awareness of personal responsibility for the own fate as key principles of the "Self-made-man" concept. O'Neill's central concern here is to explicate Kant's account of how we could have unconditional practical reasons to do as morality requires, where unconditional practical reasons are those not based upon arbitrarily chosen ends. But then, what is their basis? Kant's proposal, O'Neill argues, is that what makes a practical reason unconditional is its universal recognizability. An unconditional practical reason is one that can be seen to be a reason for action by any rational audience; its appeal relies on no (...) parochial concerns. Such universal appeal is captured by the categorical imperative test : only principles of action that pass this test can be universally recognized as yielding practical reasons. (shrink)

This essay asks what it is about the practice of trade that grounds duties of justice between states as trade partners. The answer advanced is that such duties are grounded in the dependence that trade generates. The essay puts forward four conditions that a plausible account of grounding in trade must meet: it must admit of degrees, explain the distinctly international character of trade justice, ground both procedural and distributive duties, and it must be a necessary feature of all trade (...) relationships which generate duties of justice. A dependence account of grounding meets all four conditions, and does so in an intuitively compelling way. While other accounts of what grounds duties of trade justice can meet some of the conditions, none can meet all of them. Relative to rival candidates, then, the dependence account provides a firmer foundation for the ongoing attempts to develop a comprehensive theory of trade justice. (shrink)

The article deals with the concept of the Russian national type of personality by G. P. Fedotov. The author finds the connection between Fedotov’s views and the theory of moderate social constructivism, according to which the formation of nation by elite can be successful only if it comes in accordance with geographical and historical ‘landscapes‘. Russian type of personality is seen as a unity of two polar characters - ‘the muscovite‘ and ‘the intelligent‘. The author points out that Fedotov considers (...) culture as a main factor in the development of history, which makes the very existence of intelligentsia the most important condition of solving many social problems in Russia. Otávio Bueno gives a positive and accurate summary of my defense of Platonism, with special emphasis on the epistemological issues. He criticizes “skeptical nominalism”, and proposes instead an “agnostic nominalism”, which treats mathematical objects as “objects of thought”, and neither rejects nor accepts abstract entities. In my response I argue that the main problem for nominalism is to account for abstract properties and relations, and that treating mathematical objects as objects of thought does not provide a satisfactory solution to that (...) end.Otávio Bueno apresenta um resumo positivo e correto de minha defesa do Platonismo, com ênfase especial nos aspectos epistemológicos. Ele critica o “nominalismo cético” e propõe um “nominalismo agnóstico” que trata os objetos matemáticos como “objetos de pensamento”, sem rejeitar ou aceitar entes abstratos. Em minha réplica argumento que o problema principal para o nominalismo é dar conta das propriedades e relações abstratas, e que tratar os objetos matemáticos como objetos de pensamento não é uma solução satisfatória para esse fim. My disagreement with the deflationist treatment of truth affects my attitude to Paul Horwich’s approach to meaning and intentionality. In my response I summarize objections to the deflationist account of truth developed in some detail in chapters 2, 7, and 12, and argue that the notion of intentionality should be treated naturalistically in a broader context than the context of the referential import of the locution “means that”.Minha discordância com a visão deflacionista da verdade afeta minha atitude em relação às (...) propostas deflacionistas de Paul Horwich para a intencio-nalidade.
